St. Matthew 2021-2022 Faith Formation and Sacraments Registration

The registration form is a “fillable” PDF. You can complete it using the text tool in Adobe Reader, Adobe Acrobat or other PDF program. Or you can print it out, fill it in, and hand it in when you come to your first Wednesday evening class. Or email the form to Janelle Rohr at

Please review our COVID-19 precautions before you register your student for Faith Formation and sacramental preparation: Faith Formation COVID prevention families (PDF file)

This registration form is only for students who do not attend Community of Saints Regional Catholic School. A separate Sacraments registration form for COS students is available from Community of Saints Regional Catholic School.

Please note: For those who would like to receive sacraments (First Communion, First Reconciliation, or Confirmation), weekly Faith Formation classes are required preparation.
